My cousin is pregnant and we are worried about the ultra sound indicating fluid on the back of the head.?

My cousin is pregnant and the ultrasound indicated fluid on the back on the head. They did some tests and ruled out spinabifida and down syndrome. They said something about 2 being normal but 4 can be indicative of heart problems. AFC test was normal. What could the possible problem be and what are long term effects? Thanks!
